To Have a Thesis Bound

Bring the following to the Library Administrative Offices (2060 HBLL Phone: 801-422-2905):

1. One copy of your thesis for every copy you want bound (ex: If you are ordering 6 copies, you must bring 6 copies)
   • Each copy must be in a separate manila envelope (we have extra envelopes in our office if you need them)
   • We are not involved in the specifications for margins, fonts, types of paper, etc. Please direct questions concerning these guidelines to your department or the graduate studies website.
2. An extra copy of the title page and abstract (This can be on any type of paper.)
3. Your completed 8D form
   • Ask your department or graduate studies for a blank form
   • Personal copies are sent to the address on this form
   • Department Approval: You MUST have BOTH signatures
4. Payment by check, exact cash (we don’t have change), credit or debit card
   • Make checks payable to BYU

To Have a Dissertation Bound

Bring the following to the HBLL Business Office (2062 HBLL Phone: 801-422-2905):

1. One copy of your thesis for every copy you want bound (ex: If you are ordering 6 copies, you must bring 6 copies)
   • Each copy must be in a separate manila envelope (we have extra envelopes in our office if you need them)
   • We are not involved in the specifications for margins, fonts types of paper, etc. Please direct questions concerning these guidelines to your department or the graduate studies website.
2. An extra copy of the title page and abstract (This can be on anytype of paper.)
3. Your completed 8D form
   • Ask your department or graduate studies for a blank form
   • Personal copies are sent to the address on this form
   • Department Approval: You MUST have BOTH signatures
4. Your completed UMI form (must be signed on the back)
5. Payment by check, exact cash (we don’t have change), credit or debit card
   • Make checks payable to BYU

Thesis and Dissertation Fees

• $15 per copy (four copies are required by graduate studies, you may make as many additional personal copies as you need)
• $55 microfilming fee for all dissertations
• $45 optional copyright fee, available only for dissertations
• Fee required to be mailed outside of the U.S.:
  $20 extra for air mail per copy
  $6 extra for surface mail per copy
• CD covers or pockets can be inserted for $3 per copy
